How To Change Your Name
Social Security Name Change Application
You must have a marriage license prior to wedding.
First, Click the "Certificate of Marriage" photo to complete the Indiana License Application.
Next, go to your county's clerk of courts office to pick up your marriage license. Marion County Clerk Office phone number is 317-327-4740 and is located at 200 E. Washington St., Suite W-122, Indianapolis, IN 46204.
Finally, once you have your marriage license, you can contact us via call or text 317-300-4416 to get married in our office.
*PLEASE NOTE- If you are an OUT-OF-STATE couple who is planning to get married in the State of Indiana, you WILL NEED to obtain a marriage license from the county in which you are planning to marry in.
If there is a change to your name, you will need to visit a BMV branch and apply for an amended license or identification card within 30 days of the legal name change. Name changes cannot be completed online. You must visit a Social Security Administration office to officially change your name on Social Security documentation, and allow at least one business day after your name change transaction is completed before visiting a BMV branch.
Acceptable documents supporting a legal name change include, but are not limited to:

Social security administration name change after marriage
To change your name on your social security card after marriage, you need to:

Get your marriage license.

Fill out the Application for a Social Security Card (Form SS-5) and bring it to a local office along with unexpired identification and proof of name change, such as a marriage or divorce document, adoption decree, amended or corrected birth certificate, court order with the name change, or tribal document.

Mail or hand-deliver the application to your local Social Security office.
